This form is for letting me know how many people are interested in the new Club DSM T-shirts, based on the Bay Area Tshirt that was printed. It has a pic of the BA DSMers taking over Lombard street, filling the twisty section from top to bottom.
The shirts will look similar to the below pics, though the back pic is a little darker than shown in order to give the proper detail to the cars. The front lettering will be about 4-5" wide on the left breast. The back is about 6.5" wide by 12 " tall. Click on the pics to see larger versions.
Cost will be $14/each plus shipping for most sizes (XXL costs $2 extra, 3XL $3 extra, 4XL $4 extra). In any case, the material used is high quality - 6.1 oz. 100% cotton preshrunk Hanes Beefy Tees, from what I've been told by the shop. Assume $3.85 shipping for up to 2 shirts if you are in the US, and $6 per shirt if in Canada - we will rebate any excess shipping costs to you if you wish (or just keep it for the Tool Fund).
NEW! Regional versions of the front logo ARE available. If you see your Regional logo on this page, it means you can order one specifically for your Regional club. Just choose which Regional Logo you want on the form. If you want both Regional and the main Club DSM logoed shirts in the same color, fill out the form listing the Regional logo, and note in the Comments section as to what sizes/colors/quantities you want of the Regional, and sizes/colors/quantity you want of the main Club DSM shirts. Note that the back will still have the Lombard pic on it.
Make SURE to choose the right Regional logo if you want a Regional shirt, as I will not be emailing each person to verify that the Regional logo they chose is really the one they want.
Only fill out the form for the number of shirts you want. If you need 2 or 3 different sizes of a color, choose one of the sizes, then choose the total number of shirts of that color wanted. Then note in the Comments section the number of each size you really need.
Once printing dates are set, I will email all the people that have filled out the form, asking them if they are still interested. If they answer yes, they'll get my address to send payments to (PayPal or Money Orders preferred, personal checks OK, Credit Cards can be used via PayPal.com). Any payments that arrive before the deadline date will get a shirt, any that arrive after the deadline can either request their payment back, or have me hold it until the next printing of shirts.
Please note, we ARE making a profit on these shirts. However, it isn't going to a person, it is going to Bay Area DSM, to help add tools to our Tool Fund (See "BAD Tools" on the Bay Area site for details).
